Information for Patients and Health Care Providers

Heading
Patients living with heart failure are often asked by their healthcare providers to consume a diet low in sodium, with some being advised to restrict their fluid intake. This can often be challenging due to barriers such as limited patient knowledge and/or food skills. The Sodium NavigatorHF app aims to provide nutrition education related to sodium and fluid. Guided by a health coach, the user completes various learning modules where they develop the knowledge and skills needed to adhere to their dietary sodium and fluid recommendations.
What will patients learn?
The HF app contains 6 learning modules: Guideline Recommendations, Dietary Contributors of Sodium, Reading Food Labels, Lowering Dietary Sodium, Fluid Restriction, and Goal Setting. Content and messaging in the app align with established HF guidelines and the 2019 Canada’s Food Guide. Each module has specific learning objectives and interactive activities to engage patients in learning and developing food skills.

Each module includes a combination of engaging content, question/answer quizzes, and game-based activities. The main menu also includes the Sodium Calculator, a validated screening tool that users can complete to assess their sodium intake; and a goal setting and tracking feature, where users are guided to make SMART goals.
What research have we conducted with the heart failure app?
The Sodium NavigatorHF app has been developed and has undergone user-testing among patients with heart failure, data that has been used to further optimize the app. Future studies will determine if the app changes knowledge, attitudes, behaviours and health outcomes in patients with heart failure.
